I give my permission for my child to attend the Northwest Fine Arts with Neighborhood Church staff and/volunteers. Neighborhood Church will transport kids to and from the event. I understand that I will need to sign the Annual Medical and Photo release form available on our website, if I have not already done so. During the event of an emergency, I can be reached at
